Prices For Services And Appointments

  • First Visit: Evaluation - $215

    New patients undergo evaluation one-on-one with the Doctor of Physical Therapy (60-90 minutes)

  • Revisit (In Home) - $200

    All following visits including home treatments, home re-evaluations, and home discharge assessments (30-60 minutes)

  • Revisit (In Clinic) - $150

    All following visits including treatments, re-evaluations, and discharge assessments (30-60 minutes)

  • Consultation- $50

    Talk with the Doctor of Physical Therapy without hands-on testing (30 minutes)

Payment Service Agreement

Payment for services is due in full and required at the appointment. Please let us know if you require documentation for personal insurance reimbursement.
